Mission Statement:
Our mission at Rehoboth Home Support, LLC is to provide a variety of community based supports and services to individuals with intellectual and developmental disabilities through Person Centered programs defined by empowering them to live as independently and productively as possible while promoting individual choice, dignity, and quality of life. We encourage participation of family and friends in maintaining regular involvement in lives of the individuals we serve.
Residential Services/Host Home Provider
Rehoboth Home Support, LLC establishes a variety of living arrangements and habilitation as it opens its home to meet the unique needs of each person under its care.
Respite
Rehoboth Home Support, LLC is the agency of choice for families who need so much needed and well deserved breaks from the loved one they care for.  RHS will be offering high quality, short-term respite services to provide relief for those who normally provide care for the person receiving services.  Respite will likewise be offered in the event of the absence of the person’s normal care provider.  RHS will ensure that the time people spend receiving respite services will be spent doing preferred and meaningful activities at home and also in the community.
Personal Care
Rehoboth Home Support, LLC is excited to partner with people to provide outstanding service supporting people to complete necessary activities of daily living.  RHS recognizes that these activities of daily living are repetitive and will work with people to help them establish effective routines in completing daily tasks. This will be accomplished in a manner that benefits people to become as independent as possible while finding individualized methods to support people’s varying abilities and skills.
Community Connector
Rehoboth Home Support, LLC is excited to partner with families to support their child with outstanding support in local communities and Colorado’s natural areas supporting each person’s unique interests, skills, and abilities.  RHS will solicit input from each child supported as well as their family. RHS will identify goals important to them to work on while providing community connector to build towards increased independence and achieving goals, dreams and desires.  RHS will provide a vast array of opportunities to facilitate and build relationships, connections and friendships as well as building potential and emerging natural supports.  RHS will use the community and local environments as an atmosphere to learn and provide services that support identified goals in each person’s individualized Service Plan.
